This innovative online resource provides guidance and support for individuals seeking to improve their diet, exercise habits, and overall lifestyle.

Developed in collaboration with the NHS and local healthcare providers, Your Lifestyle Prescription is available online or to download and encourages small, sustainable changes.

• Putting you in control of your lifestyle changes
• By having 'Your Lifestyle Prescription' it may reduce the need for treatment and stop you having health problems in the future

Chair of Cheshire East Council's adults and health committee, Cllr Jill Rhodes, said: "We are thrilled to introduce this cutting-edge resource that has the potential to transform the health and well-being of our residents. 

"By providing accessible support, we aim to empower people to make informed choices and lead healthier lives."

Your Lifestyle Prescription helps users achieve their health goals effectively so they can enjoy a better quality and healthier way of life.

Dr Lalit Gurnani at Nantwich Health Centre, said: "I strongly believe that lifestyle changes are an integral element of self-care for all of us, and especially important for those living with significant long-term conditions.
